Red flags that mean a wisdom tooth should come out today
Wisdom teeth sit at the back where there's rarely room. One that comes in sideways presses into the molar ahead of it. One that breaks only halfway through the gum leaves a flap that traps food and gets infected again and again. One too far back to brush decays. Those are the teeth that come out.
The signals: swelling and soreness behind your last molar, a jaw you can't open fully, a bad taste from that corner of the mouth, pain that runs to the ear, or a hole you can feel with your tongue. If the swelling has reached the neck or the eye, if you have a fever, or if swallowing or breathing has become hard, the emergency room is the first stop, not our office. Hospitals treat spreading infection and injuries; they generally don't treat the tooth itself. Then it's our turn: call us and the tooth gets handled here.
What happens at your same-day visit
X-rays first. On a wisdom tooth they matter more than usual, because the image shows how the tooth is angled, whether bone covers it, how the roots are shaped, and how close they sit to the sinus above an upper tooth or, on a lower tooth, to the nerve canal in the jaw. Then comes the diagnosis, with the price in writing.
A fully erupted upper wisdom tooth is often a simple extraction: we loosen it and lift it out whole. A lower one lying sideways under bone calls for a surgical extraction. We open the gum, trim the bone covering the tooth, section it and take it out in pieces, and a stitch or two closes the gum. Both are done here under local anesthetic. You feel pressure, not sharp pain. Nitrous oxide, oral conscious sedation and IV sedation are available; let us know when you call.

Usually, yes. Phone (859) 555-0147 during office hours and we set aside a slot that day. After the X-rays and exam, and once you've seen the written price, the tooth comes out at the same visit. When the gum around it is badly infected, a short course of antibiotics may come first so the area numbs well.
Do all four wisdom teeth need to come out at once?
No. At an emergency visit we remove the tooth causing the problem. The X-rays also show the other three, and if any look likely to cause trouble, we tell you and talk about timing. Some people have the rest removed later; some never need to. That's a decision for a calmer day.
How long does it take to recover from a wisdom tooth extraction?
Expect swelling and soreness that peak around day two or three, then fade over about a week. Lower surgical sites heal slower than uppers. Keep the gauze in until bleeding stops, avoid straws and smoking for several days to protect the clot, eat soft food, and take pain medication as directed. If, past day three, the pain is climbing rather than easing, call us.
